Another Texas town, El Paso, implemented ordinances such as distance rules a couple of years ago. It really did not change anything, except at a few clubs.

Hard to pick one "hottest," but here are some hot ones:
"The Name of the Rose" (1986) - Valentina Vargas uses Christian Slater for her pleasure. It's a very naturalistic scene, and looks like Slater was really surprised by what Valentina was doing to him for the scene.
"L'Amant" (1992) - Jane March rides Tony Leung ka Fai with abandon.

I don't pay much attention to the music, but avoid two kinds of music in clubs: Songs cut short, and gangsta-[c]rap.
If I were choosing a song for a lapdance, I guess I'd pick the unabridged "Scandalous Sex Suite" by Prince. Over 20 minutes of good rhythm! Obviously, no club would go for that.

One club I've visited (Foxy's in El Paso) has the waitresses dance onstage topless, but they are not allowed to give lappers. Each waitress I've seen there has been better looking than the dancers.

This customer agrees that your list of don'ts is reasonable, and should be common courtesy.
Another customer mistake that surely must turn off dancers: odor. Clean up! No cologne is necessary (and may be a turn-off itself), but BO is repulsive to everyone. And bad breath is something to avoid as well. Brush, floss, rinse, and use gum or mints if needed.

I could give you a gauge for VHM an UHM, but Vegas is a poor reference, as the mileage there does not typically merit either descriptor.
VHM would involve unrestricted touching, and likely intimate kissing.
UHM is FS, or BJ if that's as far as you want to take it.

Outlook on dancers: I try not to generalize.
Paying for talking: Negative.
Buying drinks: So long as they are reasonable prices, not like "dancer drinks."
Expectations from VIP or LD: I take no expectations that are not explicitly given in advance; I do take high hopes, regardless.
Extras: Over half of the dancers I have encountered would do extras, but I tend not to inquire directly...just let it happen.

The situation you were in, once she was willing to take it to management rather than accept your offer, was one in which you had no control. You could have refused to pay more than a certain amount (as you did), and they could have tossed you for no reason other than it's their choice. You ended up at the mercy of the manager, and it sounds like he tried not to lose a customer and not to make the dancer face any consequence for trying to rip you off. Probably about the best result you could expect.
