Amen, yoda. Basically, if she doesn't out and out offer it, they usually don't. Like I said, if you're in places like H-Town or Dallas, don't even bother.
In those two aforementioned towns, you can ask. The ones that don't play usually won't be offended that you did ask (they know the situation in those clubs anyway.)
The rest of the time in those clubs, you can pretty much literally see who plays and who doesn't, and that solves all your questions.

I'll admit if the dancer is hot, and I have some $$ to spend, that I'll check out the menu if it's offered. But, I don't initiate the contact or the rules; I always make sure the dancer sets the limits on what she'll allow. (I admit I have done some CFS in H-Town in the past, but always with a raincoat.)
I've had fun at clubs where I knew many of the strippers 'like to play', as they put it, but without going for FS. Most of the time, that's how I approach getting a lap dance.
For example, I was in Houston Saturday afternoon at Treasures and I decided to get a set of 2 dances from a hot looking dancer. Now here comes the odd part: she asked me after the two songs "Did I want to play?"

Besides, even though I had a raincoat available, I didn't go to Treasures to f++k, when you can go to spas for less cash for the same thing.

Some of the clubs I go to use dark lights, but if they wanted to stop the 'good times' all they had to do is get more security.
For example, in some towns in Texas (especially Dallas and Houston), it is well known that if you have the $$$, you can get the full menu (wink, wink). A lot of clubs in those towns know that strippers are doing hand jobs and blow jobs and having sex in the clubs, but they don't give a damn as long as 1) the club gets a cut of the cash, and 2) LE doesn't know about it.

I'm not sure out of this group who would give high quality dances if they were strippers, but I think that Catherine Zeta-Jones and Eva Longoria would probably be air dancers.

I went to a club in Texas once where the dancer that was dancing for me was around 37. She proved it by talking about stuff that happened in the late 1970's through 1980's, which most people under 30 have no clue about.

Anyplace North of Detroit in Michigan, ie Flint or Saginaw.
Flint and Saginaw offer full nude clubs, but no alcohol is served, there is limited touching, and the girls are usually about a 6 on average (and I'm being kind!!)
If you're in Michigan, go to the Flight Club in Inkster, right near Metro Airport. The dancers are nearly as pretty as the ones in Vegas and Texas, but the worst you'll get usually from the dancers is tits in your mouth to suck on and an outside the pants hand job.

As far as "Most Top 40 Clubs Visited", I've been to these: Treasures (Houston, Tx) and Flight Club (Inkster, Michigan). Both are very worth their reputation, though expensive.
I've been to Landing Strip in MI, and its a nice second tier titty bar, way above any Deja Vu or local dive, but below the Top 40 Clubs.

Mid 20's to early 30's are my preferences.
Anything outside that range for me is usually not too pleasing to me at all (of course, there are exceptions. Body matters more. I like to look at a body that has a tight ass and nice tits to suck on.
An average face and a tight body is what I'd prefer, but if you can get a pretty face and same for the body, that is ok, but in strip clubs, that type is usually stuck up.
It's when you get a tight body, pretty face, and nice personality is when you hit the jackpot.

Wow, you guys lead very interesting lives.
