High mileage is anything that's more than a no frills grind on the pants. That's a lapdance, plain and simple.
For example, two way contact to me is high mileage because there are places where guys aren't supposed to touch dancers when getting dances on the floor. It's on the low end, but still high mileage. We don't throw around VHM and UHM much these days, but that's why those terms exist. VHM is basically stuff up to FS and UHM is FS and above.
There was someplace I got dances where the girls had to dance on a little fucking table the whole time. Feet couldn't touch the floor at all. I think it was around Denver, but maybe not because Denver had crazy high mileage back in those days. Defining things like "high mileage" and "extras" is about as close as TUSCL gets to philosophical questions, which basically means they can't really be answered. To me, lots of mutual contact as well as grinding and stick shifting is "high mileage". Extras territory starts at BJs and HJs. LDK (look it up in the glossary) lies in the demilitarized zone between "extras" and "high mileage".
But these are terms that barely (or really don't) matter anywhere but in online discussions. I don't know of anyone who has ever walked up to a dancer and asked "What extras do you offer?" or "Are you high mileage?" Also, what is considered "high mileage" will vary wildly by region or by club.

Mileage is mileage and extras are extras.
Extras are intended to get you off and would bring disrepute on the club if broadly known.
To me, medium mileage is a good grind, touching everywhere except the pussy.
High mileage is that plus kissing, sucking titties, fingering any hole, but not extras.
VHM and UHM are a synonym for extras.
It can be relative to the geographic area, e.g. any contact in DC used to be high mileage and skin on skin would be UHM. I’d say the bare minimum is two way contact for high mileage no matter where you are and VHM or UHM begins at skin and continues through bodily fluids. As always, your mileage may vary (YMMV) and it also appears the definition will vary as well.
Most of the guys who patronize strip clubs look at the women as vending machines.

Extras bring disrepute to a club if broadly known. Seriously Tet? Extras at Desires are about as secret as the sun rising in the east. I am not sure they are held in disrepute.
Desires seems to have an "in". Haven't been on a weekend, but they used to have cops there and no one cared. The Zebra had other issues and basically no class when it came to extras with a stupid line.

I don't really get how unless it's done to the customer it's not "extra". If something a customer has penetrates a dancer below the belt, that's an extra the way I see it.
To answer the OP's question, I'm not a big fan of labels like that but If pressed I'd say skin to skin contact between the legs would constitute high mileage. Medium might include over the pants or panties contact, but would definitely include hands on interaction with basically everything else.

@wallanon, we all have different definitions of the term. I define it as anything meant to get you off. Fingering is right on the border (it can get her off).

I don't see much point in thinking about this. If a dancer walks up to me, and quickly offers an HJ or something, I'll mention it in a review. But I won't give the work name or any identifying description of the dancer who made the offer.
If something apparently spontaneous happens in the couch room, I don't mention that, as I'm not sure it would be generally available. From reading posts on this site, I have to think I'm more polite to dancers than average, less inclined to think I'm worth more than they are. Seems like this is a factor for "light" extras, like kissing. Never had a dancer spontaneously pull my zipper down though.

Mileage is often determined by the club-rules and/or local-ordinances – assuming these 2-factors are not in play; my coarse definitions are:
1) Good Mileage = two-way-contact dances – the dancer grinds on me for most of the song if not the whole song and I’m able to grope T&A
2) High Mileage = #1 above plus titty-sucking and her stick-shifting
3) Very High Mileage = #1 and #2 above plus FIV (finger in vagina); and maybye D.O (dick-out) dances (she grinds on my bare-johnson but w/o insertion or plays w/ my bare-johnson)
4) Ultra High Mileage – she fucks you in the dance-area (makes it look like she’s giving you a grinding dance but she's actually riding-you)
I don’t bother w/ a club if I don’t get at least #1 above – strip-clubbing is expensive and I don’t wanna pay to just get teased (air-dances; etc) – I also don’t necessarily expect anything more than #1 above, but of course the more I get the more I’ll spend/tip
