There are some (not just fuglies) that will provide for the basic rate, but they are scarce and have had a positive history with you. $100 over the basic rate goes a long way with all but the absolute top shelf. This of course is slowly ratcheting upwards.

There are girls that will dance and make $50-$150 waiting to find a VIP and there are girls that will drink some guy's free drinks for an hour or two before going to VIP. Whatever works for them.

The big money maker for dancers was $1 bills in the garters. Girls would rotate maybe once every 90 minutes and get the equivalent of $100 or $200 stuffed in their garters.
