I had one waitress i was flirting with give me the number for carryout at the resturant she worked at. Later she set me up at the bar to get whatever i wanted for free. As many drinks as i could drink.
Usually I don't ask; they just give it to me if they want me to come back, so no, I've never got a wrong number.
I did ask for my CF's, She changed her schedule and I kept missing her. A reverse search even turned up her real name so she must not do it often! Most just say "wireless". I already knew her first name but now I can cyber stalk her properly. As a bonus she even voluntarily sent some pics...
Yes, girls have given me wrong numbers. Sometimes I have encountered them again, like at the place I originally met them. Usually there is no way to further engage with them. Oh well.

If there is a number I want I enter it in my phone and call hers while she is sitting with me to make sure we are connected. I let her know if she does not want to connect later don't return my text, no big deal. I tell her its just to see when she is working and leave it at that.
I guess it happens when a guy is too aggressive in trying to get the girl's number. Oh well, sometimes it can't be avoided. Sometimes the opportunity is limited, so either you are aggressive, or you lose the chance.

There was a radio station here in Phoenix who gave out a number to girls who were going out partying for the weekend to give to dudes who hit on them but they weren't interested in. It was to a voice mail number at the radio station. Then they would play the calls of the guys the next week on the air. Sometimes the guy would call 4-5 times either sounding more pathetic or pissed off each successive time. It was pretty funny.
