So as you get familiar with a dancer, and move toward brief hand-in-the-pants episodes, or some bare-wood-on-the-belly-or-back grinding during a song ... Do you consider that part of the bargain for seeing her more times ... and stay to the agreed dance price? Or do you tip above?

You should tip, if you are gaining preference for her dances also.
If she is only managing to keep your attention, but you have your eye on other girls, and wouldn't mind if she didn't keep giving you dances, then don't tip above.
If she is showing some extra appreciation bc she made you wait or something, you don't need to tip above.
Depends on my mood, but it's likely that I'd tip a bit extra.
I don't like to waste money, but this is not a hobby for cheapskates.
I wouldn't waste any more money on that sort of dance. It'd be one and done.
Keep 'me coming back and offering more with larger tips.
