Just now I was gonna reject a review - selected "Not Enough Details" and even wrote some suggestions in the text box but still hit the damn Publish button instead of Reject - f'ing-a

I have both, maybe there should be a two step authentication.
^ I was thinking the same thing but even then muscle-memory may kick in but it would still help
Shit happens. :)
It's not the end of the world if a bad review gets published. The site will survive if it happens every once in a while.
