I have the most experience with Chicago prices. The OP's prices are spot on. I'll post some lows:

ITC BJ $80 (it's a hard sell)
ITC FS $200 (not as hard, but still requires effort)
OTC FS $500 (this seems to be the starting number for like forever, I never partook, but girls will quickly go down to $350, GFE (at least in the few times I've asked) off the table

At Scores and 390, usually the bouncer needs to be tipped too, at least $20, girls might say $50. This is when you use the VIP room. It's frowned upon to use the lap dance area so be discreet. Nothing will happen to the guy, but the girl will most likely be fired.

At Polekatz, supposedly if you get the hour and the add-ons (bottle service), FS can be had, but you're already at like at least $500 (also tip the bouncer and waitress). This was told by me by girls who used to work at Polekatz.
