My wife and I would like to go to TJ in the near future. Are the establishment welcoming of couples? How would the girls react to bringing a female? Any input on personal experiences and recommendations are appreciated.
Also, from what I've read already, it sounds like the alcohol flows like water TJ. I don't drink anymore. Are there mandatory drink purchases?

I was just there this past weekend and saw 3 couples and 2 out of the 3 the wives were just as into the club as the men were - in that they were tipping the dancers, engaging with the meseros, etc.. just like you see couples at a club in the States.
The 3rd couple - the wife/gf did not look like she was having a good time and by extension neither of them were having a good time.

Thats totally understandable. When dealing with a couples situation, the tone can change if a line is crossed and we get into our feelings. Jealousy, self worth, resentment etc. We've visited the Nevada brothels and had fun. I knew their prices were high but talk about sticker shock. For you - it would be your choice. Presumably, you'll have a room at Cas or Rizo, and if you want to invite a chica to shower first in an arriba, it's your time and that's up to you. The hotels will even provided these little shower caps for the ladies if they use the shower or Jacuzzi and don't want to get their hair wet. IMHO, a dip in the Jacuzzi is a totally worthwhile way to get cleaned up (and those Jacuzzis are huge, can definitely comfortable fit a trio.)

As far as chicas, a wingman has a story about a chica who used a flashlight to inspect his junk before the action started, but I think that's relatively rare, most times she is just stealthily eyeballing your shit when she goes down on your or puts the condom on. That really depends on what you have in mind. Many girls will do trios but not girl-on-girl action.
Some girls will definitely do girl-on-girl (DATY, etc), some will even be into it. If you're judicious during the ficha process you will definitely find one that's into the idea of a trio with girl on girl action, or at least pretends well enough to be into the idea.
Though I've read exploits from other mongers about bringing wives (and SBs), I've only rarely have I seen couples there in person. I've opined before that Hong Kong club is a level 70 club. tuscl.net I think this applies to both you and your wife: you should both be armed with the constitution, endurance and other mongering skills to handle a weekend in TJ.
