Others have described the layout, so I won't. The main thing to know about this place is that it is not (necessarily) a "regulars" place. It's A no hussle place. The dancers aren't supposed to approach you unless you show interest. Make extended eye contact; walk up to them; smile; drop a bunch of bill during the stage show; something. I went out of my way to malls eye contact with several of the women and they each dropped by.
Extras are the norm in the CR, and typically don't cost anything beyond the cover price: $30 to the house, $5 tip to the bouncer (to leave you alone) and $100 to the woman. The menu varies by dancer. They're willing to talk specifics if you are (gently) insistent before you go in. Don't take "you'll enjoy yourself, trust me" as an answer if you're looking for something beyond the basics. Most are not interested in going beyond a HJ, so be sure to ask.
The club opens at noon, but the dancers are not necessarily all there by that point. When I went Tuesday at noon there wasn't a single dancer ready until 12:30.
