HK is coupled to Hotel Cascadas. If you want to take a girl 'arriba/upstairs' you have to pay for the room. You have two options; book your own room, or rent a short-term room for $17/half hour. The benefit to having your own room is the following; 1) you don't get pestered every 30 minutes that your time is up, 2) depending on the night and number of girls you take upstairs booking your own room is significantly cheaper. For VIPs, the nightly rooms are 80% off on Monday and Tuesdays, and 20% off any other day. So, a single cost me $24 for two nights. 3) obviously, if you are staying multiple days or just need to rest between sessions.
HK and the Hotel are adjacent to each other, but you cannot get to the other without going outside. So, they give the girls robes so they are not half naked on the sidewalk for the literal 10' walk between the two. The robe rental is $1 plus a $1 tip for the 'robe' guy' which is just one of the 'bouncers' at the entrance to HK.
I cannot comment on the extended time ... haven't done that. Reading through several of the older reviews should give you an idea. I recall reading one that suggested they track the girls time upstairs and check-in with them after each hour for safety, I presume. But, if the girls don't go up past the lobby desk they may be able to get away from that. I have also read that HK also owns the club and hotel across the street, so you can take girls over there and vise-a-versa. However, you cannot do so with ladies from Adelita's. It is unclear to me if you can get street girls (SGs) up to your room at Cascadas. Several reviews from Adelita's say that guys like to grab a SG in the alley, but again, it's unclear where they take them for the deed.
The HK has a gallery of photos on their website and post several on their twitter account. A free login to the HK website will get you access to the pictures. They are VERY real/accurate from what I saw. Of course, some of the girls that I saw in the brighter lights of the hotel knocked them down a notch or two. As always, Google is your friend. Some guys have even posted video of their private time with the girls.
