I was there twice last month and there was a remodel. It was clearly cleaner and much nicer then my visit about a year ago. I spoke with the dancers and they confirmed the remodel. Maybe it was $12 or $10 - regardless I paid a flat fee at the door (I remember it as 10) and it included 2 drinks. This is despite them saying "no cover". As to names, I'm horrible with names.
