Roll Call + Free Entry Form

Have any of you filled out the Get Free Entry form on a dancer's profile to "book" her for a visit? There's a lot of info I don't really want to put on there; sure I have a GVoice phone number and a SPAM email address, but still, putting that info on such a site gives me pause.
